Unzip my supplies. Install the brush in your brushes folder the mask into your mask folder.

Lets start!
Open a new image 500 x 500 and floodfill with white.
Name this layer background.
Selections > Modify > Contract.
Contract by 50
Selections > Invert.
Hit Delete.
Selections > Invert.
Add a new raster layer.
Selections > Modify > Contract.
Contract by 20.
Select a dark colour from your tube and floodfill on the new layer inside the selection.
Selections> Select None.
You should have something like this
Go to Layers > Load/Save Mask > Load mask from disk.
Find the mask I supplied in the drop down box or one of your own choice and apply your with these settings;
Right click on the mask layer and hit delete. Say yes to the pop up box!
Go to Layers > Merge > Merge Group.
Make sure your merged layer is the active one.
Selections > Modify > Contract.
Contract by 70.
Take your selection tool.
Apply these settings.
Draw out a rectangle in the middle of your image, it should look something like this;
Hit delete once. This will allow the white background to show through and the feather will give it the misty effect.
Go to selections > Select none.
Highlight your white background layer in the layers palette, copy your tube and paste as new layer. Resize to suit and with your mover tool, push her to the right (Or wherever you want to put her!)
Highlight your white background layer again and add a new raster layer.
Select two complimentary colours from your tube and then apply your brush to this new layer.
Activate your tube layer and apply the following drop shadow;
You should now have something similar to this.
In your layers palette, highlight your merged layer. Set your text tool to these settings.
Set your foreground to null and your background to the dark colour you used for the fill earlier.
Apply your text to the top white border area.
My name fits nicely typed seven times across but you will probably need to adjust this depending on the length of the name you are putting on it.
Apply your text and, if you need to, pull the vector nodes in or out to make the text fit better. You want to leave a little bit of space at either end in case the text overlaps.
When you are happy with the text placement convert it to a raster layer.
Now, duplicate your text layer. Hit Image > Flip
The text is backwards!
Hit Image > Mirror and this will correct itself!
Duplicate the bottom text layer.
Image > Rotate > Free Rotate.
90 Degrees to the right..
Duplicate the rotated layer.
Image > Mirror
Image > Flip.
You should now have something like this;
Turn off the white background layer.
Layers > Merge > Merge Visible.
Turn your background layer back on and ensure it is the active layer.
