For this tutorial you will need the following supplies;
Paint Shop Pro (This tut done in version 8 but easily adapted for other versions.) trial version available here
There are no outside filters used in this tutorial!
A tube and font of your choice.
Mask here.
Brushes of your choice.
I used the gorgeous art work of Barbara Jensen for this tutorial.

Open a new image 500 x 500 with a transparent background.
Ok lets get started!
Open a new image in PSP 500 x 500 with a transparent background.
Floodfill with white.
Add a new raster layer and select a pale colour from your tube.
Floodfil new layer with the pale colour.
Layers > Load / Save Mask > Load Mask from Disk.
Locate sc-edge101 in the drop down menu and apply to your image with these settings;
Right Click on the mask layer and hit delete.
Say yes to merging the mask into the layer below.
Layers > Merge > Merge Group.
Go to your preset shapes tool. Select rectangle with these settings;
Set your foreground to a dark colour from your tube and your background to a lighter, contrasting colour.
Draw a rectangle on your image roughly this size;
Convert to raster layer.
Take your magic wand and click once on the centre of your rectangle. You will get marching ants on the inside of your border.
Go to Selections > Modify > Contract.
Contract by 5 pixels.
Floodfill the selected area with the darker colour, the one you used for the edge.
Selections > Modify > Contract.
Contract by 5 Pixels.
Hit Delete once on your keyboard.
Selections > Select None.
You should now have something like this;
Name this layer Frame 1 and duplicate once. Rename the duplicate Frame 2.
Make your Frame 2 layer active and go to Image > Rotate > Free Rotate.
Rotate the frame by 46 degrees to the right, all layers unchecked.
Position the two frame layers like this;
Make Frame 1 the active layer. With your magic wand click once inside the frame.
Selections > Modify > Expand.
Expand by 3 Pixels.
Add a new raster layer and select a swirly brush. Apply brush strokes to the selected area alternating between the dark and light colours you chose for your frame. Be creative and see what kind of cool effects you can get. When you are happy with how your brush background looks deselect.
Move the brushed layer below the frame 1 layer and lower the opacity to around 52.
Duplicate your tube by hitting shift D.
On the duplicated image go to Image > Mirror.
Resize by 150% with bicubic resample and all layers checked.
Edit > Copy.
Back on your tag, activate the Frame 2 layer and click once inside the frame.
Edit > Paste as new layer.
Move your tube layer over the selected area and position so that you get her face inside the frame.
Once you are happy with the placement, make sure your pasted tube layer is active. Go to Selections > Invert and hit Delete once. Move this layer below the frame 2 layer and lower the opacity to around 34.
Add the following drop shadow to Frame 1, Frame 2 and your tube layer.
Merge all layers visible.
Paste your tube as a new layer and resize if necessary. Position the tube towards the bottom left of the tag.
Add the following Drop Shadow;
Now for the text. I used Balcony for the base text all caps, size 72 and vector checked.
Select a darker colour for your background from your tube, set the foreground to null and apply your name.
Rotate text 90 degrees to the left and use the nodes to pull the the text to fit the edge of the tag.
Convert to raster layer.
Lower the opacity to around 50.
Now select a script font. I used Lainie Day with the same settings.
Add your name once again and rotate to position above the first layer of text. Pull the nodes to make it fit and convert to raster layer.
