Open a new image 500 x 500 with a transparent background.
Open your tube and copy it.
Paste onto your canvas.
Duplicate tube layer twice and X out top two layers.
Activate original tube layer.
Go to Effects > Plugins > Simple Filters > Horizontal Grain.
Activate next tube layer. Resize by 90%, all layers unchecked, bicubic sample checked.
Go to Effects > Plugins > Simple Filters > Top Left Mirror.
Reduce this layers opacity to 60
Activate top tube layer.
Repeat resize twice and add a drop shadow with the following settings;
Activate Horizontal Grain layer. Using your selection tool select around tube as shown below.
Selections > Invert. Hit Delete once and keep selected.
This is what it should look like;
Using your eyedropper tool, choose a dark colour for your foreground and a lighter one for your background from your tube.
On your colour palette set your foreground to gradient with the following settings;
Flood fill the selected area with your gradient. Leave selected.
Go to effects > Texture Effects > Blinds and apply the following settings;
Deselect.
Go to preset shapes and choose rectangle with the following settings;
On your color palette make your background null and keep your foreground set as gradient.
X out top 2 layers and make sure the woodgrain layer is still active.
Carefully draw a rectangle around the light area of the layer. Pull the edges of your rectangle outwards on the longest side to show a gap and then convert to raster layer.
Make all layers visible then merge visible.
Crop off any excess around the frame and then add new raster layer and send it to the bottom. Resize merged layer by 75% bicubic sample checked.
In your colour palette click on your gradient and set repeats to 50. Floodfill bottom layer with gradient.
With bottom layer still active go to your selections tool and select around merged layer leaving an edge of gradient all the way around it to create a frame.
How large or small you make the border is up to you, whatever you think looks good!
Selections > Invert.
Hit delete once and deselect.
Effects > 3D Effects
Apply the following Inner Bevel;
Deselect.
Reduce the opacity of your bottom layer to 64. Add new raster layer and send to bottom and flood fill again with your gradient.
Go back to selections and repeat the above step, to create another edging around your tag.
Merge all layers visible.
Image > Resize > Resize by 85% with bicubic resample all layers unchecked.
Add a new raster layer and send to the bottom. Floodfill this new layer with white.
Back on your merged layer, add the following drop shadow;
Change your Foreground and Background back to original colours.
Activate your text tool and apply your name with the following settings.
Position your text where you want it and, once happy with it convert vector text layer to raster layer.
Apply inner bevel and drop shadow with the same settings as used earlier to your text.
